ATHLETIC HALL OF FAME NOMINATION FORM

ATHLETE: Eligible 5 years after High School Graduation. Nominee must have attended Nazareth Academy for a minimum of 2 years, or 1 year with specific accomplishments, and must have achieved significant honors as a Nazareth Academy Athlete with consideration given to post High School accomplishments.

COACH/ADMINISTRATOR: Eligible after 20 years of involvement with athletics; 10 of the 20 years must be at Nazareth Academy. Nominee must have been an inspirational leader to the students, having made significant contributions to the Nazareth Academy Athletic program.

FRIEND OF ATHLETICS: Eligibility determined by the nominee’s longtime, outstanding support and loyalty to Nazareth Academy Athletics.
